Below is a summary of the forms you will need to complete and how we use the information. You will need to login to your Admissions Portal to access the forms, which are due June 1 unless otherwise noted.
We use the information on this form to help us make roommate and housing assignments. We cannot complete your housing assignment without it.
The information collected helps us assign a faculty advisor and helps your advisor work with you.
You may authorize Kenyon to release billing invoices to designated individuals, such as a parent or guardian.
Due July 15. You will download this form from your admissions portal and mail it to the Health and Counseling Center. We cannot issue room keys at Orientation if you have not returned this form.
This form includes instructions for submitting a photo for your K-Card, Kenyon's student ID card. Please submit a photo as soon as possible to avoid delays accessing your residence hall on move-in day. You will also need your student ID to complete any placement exams during Orientation.
We use the information on this form to contact parents in the normal course of business and in case of an emergency.
You are required to review and acknowledge policies governing computing at Kenyon and network access.
In August we will send you an email with a link to an alcohol and sexual assault education site. You will need to complete this training before registering for courses.
Under FERPA, Kenyon is permitted to disclose information from your education records to parents if they claim you as a dependent for federal tax purposes.
If you have a documented disability, please use this form to register with Student Accessibility and Support Services to receive services.
We will provide you with a free Kenyon t-shirt at Orientation. The t-shirts are cotton unisex shirts and available in sizes S-3XL. Use this form to request a size.
Please complete this form if you are interested in joining a musical ensemble or taking individual lessons.
If you are considering taking science and mathematics courses, please plan to attend the luncheon.
