Enrollment Verification Request- How to

HEALTH INSURANCE

Many health insurance companies require proof or verification of full-time enrollment to continue coverage for dependents over the age of 18 or 19. The Registrar's Office can provide this verification for each semester after the 10th class day of each semester.

If you would like to request an enrollment verification, please fill out this form and fax (740-427-5615) or mail it to the Registrar's Office.

Please note: We are unable to call and confirm receipt of your request. If you would like confirmation, you will need to call the Registrar's Office at 740-427-5121.

AUTO INSURANCE

Any verifications for auto insurance "good student" discounts that request that we provide the student's grade point average (GPA), require the student's signature on the form.
