Faculty Forms
Course-proposal form(CPC)
When proposing a course, please read and follow the instructions at the top of the form.
Submitted forms will be sent by email to you, the department chair, and the Curricular Policy Committee. CPC will not consider a proposed course until the department chair submits support.
Special Topics Notification Form
Fill out the form (one for each special topic) and save to your computer. Then attach the completed form to an email to send to the Chair of CPC and the Registrar.

OCS Override form
This form should be used to provide permission for an OCS student for web registration. This must be received in the registrar's office 24 hours before the scheduled registration period begins.
PI Course - List Override Form
This form should be used when you have several students that need an override for the same course. This is for PI courses only.
If your course is not PI, you have an additional student that needs an override, or you have already submitted this form for a particular class , you should print out and complete the single override form.
