Participation is optional and coverage is effective on your date of employment. The EBMC plan is comprised of a Preferred Provider Organization.

For the Low Plan, most expenses are subject to a $500 deductible per person per benefit year which begins July 1 and ends June 30. The maximum family deductible is $1,000. After the deductible has been met, the plan pays 70% of in network covered expenses and 50% of out of network covered expenses. The individual pays the remaining 30% or 50% of expenses. Once a covered person's out of pocket expenses total $3,000 (in network), $6,000 (family, in network) or $5,000 (out of network), $10,000 (family, out of network) in a benefit year, the Plan will pay 100% of the covered expenses incurred by that person during the remainder of the benefit year. The lifetime maximum for each covered person is two million dollars.
