Qualifying Event
An eligible employee who declined this coverage for himself or his dependents during the initial 30-day eligibility period may also enroll for coverage at a later date if the following conditions are met:- The eligible employee (and/or dependent) loses coverage under another group health plan or other health insurance coverage that was in force at the time this coverage was initially declined and was the reason for the declination. Special Enrollment is available only if it was indicated on a completed and signed Waiver of Coverage Form that other coverage was the reason for initially declining participation in this Plan.
- The loss of coverage is due to one (1) of the following events:

- Loss of eligibility for coverage as a result of cessation of employer contributions, legal separation, divorce, upon completion of a Declaration of Termination of Domestic Partnership, or due to a spouse's or domestic partner's death, termination of employment, or reduction in the number of hours employed. Loss of eligibility does not include any loss due to failure of the individual to pay premiums on a timely basis or termination for cause; or
- COBRA Continuation Coverage has been exhausted.
A Special Enrollment Period is also offered to any eligible employee (and/or dependent) who previously declined coverage for any reason who later acquires an eligible dependent (or additional eligible dependent) due to:
- Marriage
- Entering into a Domestic Partner Relationship
- Birth of a Child
- Adoption or placement for adoption
In the case of enrollment during a Special Enrollment Period, the employee must request coverage as outlined in this section within 30 days of the date COBRA Continuation Coverage has ended; the other coverage is terminated due to loss of eligibility; or of acquiring an eligible dependent. The effective date of coverage obtained under a Special Enrollment Period will generally be the first of the month following the date the completed request for enrollment is received by the College's Human Resources Department. However, in the case of marriage, domestic partnership, birth, adoption, or placement for adoption, the effective date will be the date of marriage, the date a signed Certification of Domestic Partnership is received by Human Resources, or the date of birth, adoption, or placement for adoption.
