Use this form to request a classroom change for the entire semester.
Course-proposal form (CPC)
When proposing a course, please read and follow the instructions at the top of the form. Submitted forms will be sent by email to you, the department chair, and the Curricular Policy Committee. CPC will not consider a proposed course until the department chair submits support.

CPC Course Change Request form
Request a title or number change for an approved course. Directions: Download to your computer. Fill it out. Save as a new file. Send the completed form to the chair of the Curricular Policy Committee and the Registrar.
Progress Report form (Academic Advising)
This form requires you to sign in. If you are off campus, please go to remote.kenyon.edu first.
Please submit for students on Conditional Enrollment at the end of the first for weeks and as prompted and any students who are having academic difficulty (C- or below).
Special Topics Notification Form (CPC)
Save the form to your computer before filling it out. Please fill out on form for each special topic course. Send the form to the department chair to be submitted to the Chair of CPC and the Registrar. Mac users, please be sure to open this form with Adobe Acrobat.
Web Registration Override Form (only for PI courses)
Use this form for student overrides on permission of instructor courses. If your course is not PI, overrides will not be entered.

Use this form to request an electronic waitlist for a course.
